feat(web3): 数据库连接 Tab 缩减 + 弹窗 form-grid 修复

- 任务弹窗「数据连接」Tab 数据库选项 6→3：保留 MySQL / Oracle / 达梦 DM，移除 SQL Server / PostgreSQL / JDBC 自定义
- global.css .modal 加宽 880→1000；.form-grid 列最小宽度 320px，右半边输入框不再被裁切
- docs/db-schema.md task 表 db_type 列注释同步
